ITB Syndrome or Runner’s Knee and its Best Treatment

What is ITB syndrome?

The most common cause of lateral knee pain is assumed to be ITB Syndrome. It could be ‘friction syndrome,’ in which the ITB causes friction on the structures beneath it, causing pain.   Alternatively, compression of sensitive structures beneath the ITB is anticipated, and the treatment program is tailored accordingly.

Causes:

ITBS is considered to be caused by a loss of strength and control around the hip, including weakness in hip abduction and external rotation and increased hip adduction under loading (e.g. running). Over 60% of the time, training error is thought to be a factor.

Signs & Symptoms of ITB syndrome:

ITB syndrome is unquestionably the most common cause of lateral knee pain. ITB syndrome is characterized by increased pain when running downhill or on steep routes, as well as a recent increase in training volume.

Treatment of ITB syndrome:

Treatment should attempt to initially alleviate symptoms before addressing underlying causes: there are three levels of rehabilitation programs available for this purpose, which are as follows:

  • Level 1 – Low-intensity, largely open-chain exercises (such as side-lying abductions, side-lying external rotations, and hip extension strengthening e.g. glute bridges).
  • Level 2 – Closed chain exercises with a moderate load (such as mini-squats, lunges, and step-ups).
  • Level 3 – Exercises with a higher load, such as impact and sports preparation (such as goblet squats, single leg squats, and plyometrics).
